The standard corporate response to AI ethics is a committee, a set of principles with words like fairness and transparency, and a PDF. Then the principles go on the intranet and the actual decisions keep getting made in project meetings where nobody has ever read them. The problem is not that the principles are wrong. It is that principles do not attend meetings. Questions do, if you build them into the agenda.
Why manifestos fail and checklists work
Waarom manifesten falen en checklists werken
An ethics manifesto asks people to hold values in mind. A checklist asks them to answer a question before proceeding. The second one works because it does not depend on memory, mood, or moral energy at the end of a long sprint. Aviation figured this out decades ago. Surgery followed. AI deployment is simply the next field to learn that good behaviour scales through procedure, not inspiration.
PwC's 2023 AI Governance Survey found 71 percent of organisations have a published AI policy, while only 34 percent of employees can accurately describe what it says about their role. Publication is not implementation. The gap between those two numbers is where incidents happen.
Four questions, three moments
Vier vragen, drie momenten
The whole framework fits on one page. Four questions, asked at three moments: project intake, go-live, and every review cycle after.
| Question | What it surfaces | Sign of trouble |
|---|---|---|
| 1. Who is harmed if the output is wrong? | The real stakes. A wrong product recommendation and a wrong benefits decision are different universes of risk. | The room has never discussed a wrong output, only the happy path. |
| 2. Can an affected person understand and challenge the decision? | Whether explanation and objection routes exist, in human language. | "They can file a complaint" through a process nobody has tested. |
| 3. Would we defend this use on the front page? | The gap between what is legal and what is defensible. | The plan depends on people not finding out how it works. |
| 4. Who is accountable for the outcome? | A name. Not a committee, not a vendor, a person. | Silence, or the answer is the algorithm. |
| Vraag | Wat ze blootlegt | Teken van problemen |
|---|---|---|
| 4. Wie is aanspreekbaar op de uitkomst? | Een naam. Geen commissie, geen leverancier, een persoon. | Stilte, of het antwoord is het algoritme. |
Question three deserves a defence, because it sounds soft next to the others. It is not. The front page test catches the category of harm that compliance misses: uses that are technically lawful and reputationally fatal. If the honest answer is that the project survives only in obscurity, the project has already told you what it is.
Make it bite: the pause authority
Geef het tanden: de pauzebevoegdheid
A framework without consequences is decoration. The minimum consequence: a project cannot pass intake or go-live until the four questions have written answers, and one named person holds the authority to pause a running system when an answer stops being true. That is the entire enforcement mechanism. No committee. No quarterly ethics report. A gate and a name.
Done this way, ethics stops being a parallel bureaucracy and becomes a property of how projects move. The paperwork burden is one page per project. The protection is real. And when the EU AI Act's deployer obligations arrive in August 2026, the written answers to these four questions will be most of the evidence file you need.
Expect two objections. First: this is too thin, real ethics needs depth. Perhaps, but depth that never enters the meeting protects nobody, and four answered questions protect more than forty unread pages. Second: the questions will become a ritual, answered on autopilot. True, eventually, for some teams. That is what the pause authority is for. The framework does not need everyone to care every time. It needs one person, with power, who reads the answers. Start it at the next intake meeting. Not after the framework is perfected. The next one. Perfection is how ethics frameworks stay theoretical, and theoretical was the problem we set out to solve.
Verwacht twee bezwaren.
